Even with the emergence of palliative care programs and hospice programs, the majority of elderly people do not die in their own home as is their preference. What are the reasons for this trend? Discuss what you can do as a nurse to support your clients in end-of-life care and in supporting their desires. Support your response with evidence-based literature.

Alzheimer’s is a type of dementia that causes problems with memory, thinking and behavior.

Symptoms usually develop slowly and get worse over time, becoming severe enough to interfere with daily tasks

Although the greatest known risk factor is increasing age, Alzheimer’s is not a normal part of aging

Alzheimer’s is a progressive disease, where dementia symptoms gradually worsen over a number of years. In its early stages, memory loss is mild, but with late-stage Alzheimer’s, individuals lose the ability to carry on a conversation and respond to their environment. On This presentation we will see how Alzheimer’s disease affects not just those whose memories are lost , but also those whose memories remain intact.

Fiona (Julie Christie) and Grant (Gordon Pinsent) have been married for 44 years and live in an isolated farm house in Ontario, Canada

They enjoy cross-country skiing and their intimacy but the looming clouds of Alzheimer’s disease threaten their life together in love.

In the midst of their happiest life Fiona (the wife) starts to have signs of dementia. She misplaced the frying pan away in the refrigerator , while hosting their friends for dinner, she reaches for a bottle but can’t remember the word wine, she lost track of where she is while cross-country skiing by herself

Fiona’s reminding of Grant of something she does not want to forget — the pain she felt years ago when as a university professor of mythology he had affairs with younger women left him in agony, guilt and regret from his past actions.

Grant faced the challenges of adjusting to his wife’s memory loss and her attachment to mute fellow patient Aubrey (Michael Murphy) a wheelchair-bound patient at Meadowlake

Grant , the husband will be the subject of my functional assessment to assess his functional health patterns with respect to his state of compounded loss: loss of loved one, he grieves Fiona’s loss of her past, he mourns the future they will not have together and he loses his wife to another man. Grant’s living in guilt and regret on his past infidelity and an adulterous affair also affects his transitioning & adjusting to his new way of life.

ªGrant wants to believe that these memory lapses are a normal part of the aging process and also unprecedented.

ªHe clings to the the thin thread of hope that his wife’s condition will not get worse.
