Hello Class,

Access to good health care is limited by a number of factors. These disparities include race or ethnicity, sex, sexual identity, age, disability, socioeconomic status, and geographic location (Office of Disease Prevention and Health Promotion, 2020). The biggest disparities my facility deals with is the socioeconomic status and geographic location. The latest census bureau information has 13.1% of the population living in poverty (United States Census Bureau, 2020). The majority of our patients have Medicaid, Medicare, or both. We also have a large uninsured Amish and Mennonite community.

My hospital has a financial assistance program for our uninsured patients. I am not sure how this program works, but there are discounts available for these patients. This program has been in place for a very long time and seems to work well. We do write off hundreds of thousands of dollars each year as charity care as well. We have recently started a program to help home bound patients get to their medical appointments, by using the ambulance service to transport. This program is new, and we are working to make the public aware of it. When a patient uses this service, the insurance will be billed so it is very important to have all of the paperwork completed so that insurance will cover the ambulance transport. The handful of patients that have used it have had very positive reactions.

We have several rural health clinics throughout the county so that patients do not need to travel as far for appointments. We also work with larger facilities to bring specialty providers to the patient. The nearest cardiologist is roughly sixty miles away, so bringing the specialist to our facility is a great service to the patients. We have used telehealth as well. This is used for weekend physician coverage on our inpatient floor. An advanced practice practitioner, that is local, works with an e-hospitalist to manage our observation, acute, and skilled patients. This allows us to keep more patients at the local hospital, which allows the family to be close by.

My facility uses a combination of government mandated and self-initiatives to lessen the impact of the socioeconomic and geographic location disparities.

Discussion Questions (DQ)

  • As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.
